Each Metal Tier pays a set share of costs:

Platinum: Covers 90% of your medical costs, you pay 10%.

Gold: Covers 80% of your medical cost, you pay 20%

Silver: Covers 70% of your medical costs, you pay 30%.

Bronze: Covers 60% of your medical costs, you 40%. 

The Maximum Out Of Pocket that an individual can incur is $7,350, $4,700 for a

family, that must include Deductibles, Coinsurance & Copays, not including Premiums.

Some plans have an automatic Doctor Copay for example, $10, $15, $20, $40 or $60 without having to meet the Deductible or Out Of Pocket Max.

You cannot apply for health insurance for you or your family outside of the Open Enrollment Period, which begins November 1, 2015 and ends December 15th. for a January 1, 2016 effective date , unless you have a Qualifying Event. Qualifying Events include: Job Loss, Change In Income, Marriage/Divorce, Birth/Adoption, Moved To A New State, Health Plan Being Cancelled, Expiration Of Cobra  and other Qualifying Events. If  you have a Qualifying Event you may be able to enroll outside of the Open Enrollment Period and qualify for a Special Enrollment Period (SEP).

You have choices when you shop for health insurance. Plans are organized by their level of care, Bronze, Silver, Gold and Platinum. These are called Metal Tiers. Bronze plans have the least coverage and Platinum plans have the most. If you are under 30 and have a financial hardship, you may qualify for a high-deductible, catastrophic plan.  

Each Metal Tier may offer these four types of plans:

Health Maintenance Organizations (HMOs)

Preferred Provider Organizations (PPOs)

Point-Of Service (POS) Plans

High-Deductible Health Plans (HDHPs) that can be used for Health Savings Accounts (HSAs)

With HMOs you usually have to pick a Primary Care Physician (PCP) as your Gatekeeper

and get a referral before seeing a Specialist. No Out-Of-Network option.
